Most of these patch notes are from PC updates, but it's fair to assume that similar fixes were put out for console updates as well. An Eidos employee explained that it was an error with time-stamps from March 30th - April 2nd, meaning that 'Systems didn’t recognise the most recent time-stamp and instead took the previous time-stamp save (re-starting the level).
